We are excited to launch a quick, easy, and ENTIRELY PAPERLESS process to enroll new patients on Brixadi, Sublocade, Vivitrol, HepC, and other specialty medications directly in Stream!

Simply navigate to “Provider Actions” on the left-hand side of your Stream dashboard, select the appropriate provider, click “Add Action” and click the “Enroll New Patient” button.

Follow the prompts to enter the information required. When complete, click “Save Patient Info”.
Then, upload the patient’s insurance information and medication list. For HepC enrollment, please also upload the patient’s chart notes and labs.
Send us the Rx.
Please let the patient know St. Matthews Pharmacy will be contacting them to obtain authorization. In the event we are unable to reach a patient, we will communicate with you in Stream to help ensure timely delivery.

If you prefer to use our paper enrollment forms, they are still available and can be found HERE. The completed form can be simply uploaded in Stream. Our electronic enrollment option in Stream eliminates the need for a paper form with signatures.
