1. Send the prescription to St. Matthews Specialty Pharmacy

2. Complete New Patient Referral via electronic or paper option

Electronic Referral Option: We will reach out in Stream to obtain any missing information once we have the Rx, if needed. You can also proactively complete our electronic New Patient Referral in Stream by clicking “Provider Actions” > select the provider > “Add Action” > “Enroll New Patient”.

Paper Referral Option: If preferred, access our New Patient Referral below.

Patient authorization is required for us to provide pharmacy care. If the patient is in your office, please have them complete authorization to help ensure timely shipment.
