Electronic Enrollment in Stream
Our electronic enrollment option is quick and easy through our Stream portal. In Stream, navigate to Provider Actions, click “Add Action”, click “Enroll New Patient”, and follow the prompts.

Electronic enrollment requires separate patient authorization. Please direct patients to our authorization form below and let patients know St. Matthews Pharmacy will be reaching out via text message and phone call as we must obtain patient authorization to provide pharmacy care.
